Office Space Application Procedures


You must first download the application form here.

The following information will be required:

  1. Name of organization
  2. Name of contact person from the organization, along with school address and telephone number at which the contact person may be reached
  3. Equipment needs and office requirements
  4. Willingness to accept alternative Union space
  5. Purpose of organization
  6. Benefits provided to university community
  7. Brief description of activities
  8. New activities planned for the upcoming academic year
  9. Number of university community activities presented during the past year
  10. Number of years in existence at the University of Illinois
  11. Uniqueness of activities
  12. Goals of organization for upcoming academic year
  13. Total number of organization members, active members and eligibility requirements for membership
  14. Extent of use of previously held space elsewhere on campus and reasons for wanting to be relocated
  15. Actual or proposed office hours and number of days per week the office will be open
  16. Special services provided by the organization for the university or the Illini Union
  17. Anything else that the organization feels deserves the attention of the committee
  18. Signature testifying that all the above information is true and that it will be kept up to date
